our goal is to give our clients the opportunity to live more independently, while improving their quality of life.

can provide peace of mind and guidance to both the family and the older adult.
Care Management Planning and Assessment
Determining appropriate care for a loved
one often requires time and expertise that most families and caregivers don’t have. Trying to understand insurance benefits
and care options can be a tall order for anyone. Fortunately, our care managers have experience developing and administering
care plans that meet the individual needs of older adults and their families.
Every care plan that we develop is as unique as the clients we serve. We spend a lot of time reviewing
insurance benefits, determining living or housing options, and assessing physical and medical needs. And of course, we spend
a lot of time with families and their loved ones to determine what is most important to them and their care. We ask questions
that often help with solutions that were never explored. While resources and insurance are certainly important, we believe
nothing is as important as the independence and dignity of those we serve.
